Many patients need medications customized to their unique needs: a different strength, dosage form, flavor, or ingredient combination. Our pharmacists prepare each formulation specifically for the patient in front of you, working closely with your care team throughout.
Specialties include hormone replacement therapy, GLP-1 formulations, wellness compounds, and sterile preparations including sterile testosterone.
How to send us a prescription
Three ways to send an Rx, listed in our recommended order. New compounds are easier when you call first.
Preferred
1. Electronic prescribing
Send through your EMR’s pharmacy directory (Surescripts).
- Look us up by name (Connect Wellness), phone (208) 593-0133, or NPI 1508725623.
- Most EMRs don’t have a dedicated compound entry. Enter the primary active ingredient as the medication, then put the full formulation (every ingredient with strengths, dosage form, base or vehicle, and total quantity) in the SIG or Notes-to-Pharmacy field.
- For controlled substances, including compounded testosterone (Schedule III) and Schedule II preparations, your e-prescribing software must be EPCS-certified. EPCS is the required channel for Schedule II compounds.

- For non-Schedule II controlled substances, faxed prescriptions are accepted as the original.
- We compound Schedule II preparations, but a faxed Schedule II can’t serve as the original. Send those as an EPCS electronic prescription instead.

Connect Wellness is a delivery-only pharmacy. Every prescription ships to your door, so the Eagle, ID address isn’t a walk-in or pickup location.
What to include on a compound prescription
A short reference for prescribers who don’t send compounds every day. The more specific the script, the faster we can dispense.
Standard prescription elements
Required on every Rx, compound or otherwise.

Compound-specific additions
What we need beyond the standard fields.
- Active ingredient name(s) and the strength of each (e.g., ketamine 10% / lidocaine 5%)
- Dosage form: capsule, cream, troche, sublingual drops, injection, etc.
- Base or vehicle when clinically relevant (e.g., HRT cream base, grapeseed oil for injectables). See ingredient sourcing below for our common bases.
- Total quantity of finished compound (e.g., 30 mL, 60 g, 100 capsules)
- Any flavoring, allergen, or dye exclusions

One of our highest-volume categories. Available at a prescriber's direction in the strengths and dosing schedules they specify.
Sterile compounding (USP <797> cleanroom)
Sterile preparations including sterile testosterone, prepared in a USP <797> compliant cleanroom. Many compounders are non-sterile only; we offer both.
